Commercial & Packages

To make a claim on your Business Policy, please contact the insurance broker or agent who issued your policy. If possible have your policy number available when you call. Your broker or agent will contact us on your behalf and help to fill out a claim form.


If you are unable to contact your insurance broker, please contact Allianz for advice.

Work Injury Compensation


Please submit the following if you wish to file a work injury compensation claim.


 

  • Complete the Work Injury Compensation Claim Form.
  • I-report made to Ministry of Manpower (MOM).
  • Photocopy of identity card and/or work permit.
  • Original medical bills and/or certificates.
  • Pay slips to support the injured worker's salary for the 12 calendar months prior to the accident. Earnings during the month of the accident and the month the injured employee joined cannot be considered.
  • Medical report and/or inpatient discharge summary (if applicable).

 


If the injured person is not your direct employee, we need all the relevant contracts to establish that the direct employer is your sub-contractor.


In addition, please ensure that the relevant government body is also notified.

Public Liability

Please complete the Liability Claim Form that is available in our website. You should notify us if there is any event that may give rise to a claim. A detailed description of the incident is necessary. Please provide the name of witness and their contact details. You should not admit liability at any point in time without our written consent.
