Personal Claims

Allianz makes every effort to provide you with a market leading claims service and to make the claims experience as simple and as fair as possible.

 

For personal lines products, like HomeSafe, Medical and Maid's Insurance, please make a claim on your policy by contacting the insurance broker or agent who issued your policy. If possible have your policy number available when you call. Your broker or agent will contact us on your behalf and help to fill out a claim form.

 

If you are unable to contact your insurance broker, please contact Allianz for advice.

 

HomeSafe

Please submit the General Insurance Claim Form together with the following supporting documents:

  • Purchase invoice of the loss item.
  • Purchase invoice of the replacement item.
  • Police report if there is police involvement.

 

Medical Claims

Please complete the Hospitalisation Claim Form that is available in our website. You also need to submit the following supporting documents:

  • Medical report or inpatient discharge summary.
  • Original medical invoices.
  • Referral letters.

 

Maid's Medical Claims

Please complete the Domestic Maid Insurance Medical Claim Form and mail it to the respective broker stated in your certificate of insurance, together with the following supporting documents:

  • Medical report or inpatient discharge summary.
  • Original medical invoices.
  • Original medical certificate (not applicable for outpatient claims).
  • Referral letters (if any).
  • A copy of the certificate of insurance.
  • A copy of the maid's work permit.
  • A copy of the contractual agreement between you and the maid, stating the maid's monthly salary (not applicable for outpatient claims).
  • Monthly levy to MOM (not applicable for outpatient claims and subject to the insurance plan purchased).

Maid's Bond

If you have received a letter of demand from Ministry of Manpower (MOM), we strongly recommend that you contact MOM immediately to establish the reason for the demand. If necessary, you may lodge an appeal.

 

Since you have taken up a Domestic Maid Insurance with us, you have the option of Allianz Insurance paying the bond of $5,000 to MOM on your behalf first. However, under the terms and condition of the policy, we shall recover the paid amount from you. Alternatively, you may wish to pay the bond to MOM directly.

 

Travel Claims

Please complete the Travel Claim Form that is available in our website. The supporting documents that you need to submit are listed in the claim form.
